This form is used to collect the number of members from your chapter who plan to attend the Electa Grand Chapter Call Meetings, typically held in January, March, and June.

No payment is required to attend these meetings.

Please do not use this form to register for the Annual Business Meeting.

Please list the name and birth month for each member attending.
